Healthy Opportunities Pilot Program

May 1, 2024 - Partners will launch its Healthy Opportunities Pilot Program. 

Pilot Services included addressing housing, interpersonal safety, food and transportation needs.

Qualifications:

• Have Medicaid and live in Burke and Rutherford County 
• Have two or more chronic conditions such as mental illness, cognitive developmental disabilities, brain injury, diabetes, high blood pressure
• Pregnant with high risk
• Children 0-3 Neonatal intensive care unit 
• Children 0-20 growing in an environment that is not safe and one factor of:
  
  • Homeless
  • Food insecurity
  • Transportation Insecurity
  • Experiencing Interpersonal Safety Concerns
